Appointment Information
Please come 5-10 minutes prior to your appointment.
Please bring the following:Your referral from your GP, family physician or specialist*
An up-to-date medication list Your Medicare card and, if you have one, your DVA, Pension and/or Health Care Card* *If you are unable to bring your referral and it is not in our system, we cannot process your rebate. If it is not in our system or you do not have a copy to bring at the time of your appointment, you will be asked to contact your referrer and ask them to send us a copy. *You may be eligible for concessions if you have a Pension or Health Care card Parking availability:Parking is available on both sites via Mckenzie street or on-street.

Confirmation of your appointment:The practice will send a text message or call you a week prior to your appointment to confirm your attendance. If you do not hear from us please call or email the practice to confirm your attendance.
Unable to attend? Please notify the practice [via phone (66223088) or email (cardionrcc.com.au) at least 24 hours prior to the appointment. Please note that we are an extremely busy clinic and if you wish to rebook, please inform us as soon as possible! Appointment Delays:We endeavour to keep to schedule, however, as we also provide an emergency hospital service at Lismore Base Hospital we may occasionally be called in for an emergency. Your understanding and patience will be appreciated in these circumstances.

To check the validity of your referral: check it is in date (GP referrals last 12 months after use, while referrals from Specialists only last 3 months); check it has your Doctor's name and Provider Number; and check it also is addressed to the Doctor you intend to see.
